Certification Request Form - Federal Benefits

• Students receiving the following benefits are required to submit the above form each semester they wish to receive benefits. 
  • Chapter 30 - Montgomery GI Bill®
  • Chapter 31 - Vocational Rehabilitation
  • Chapter 33 - Post 9/11 GI Bill®
  • Chapter 35 - DEA Program
  • Chapter 1606 - Montgomery GI Bill® - Selective Reserves

Hazlewood Application 

• Students wishing to submit an application for the Hazlewood Act will be required to download the appropriate paper application and upload it and all supporting documentation at the link above. 
  • Hazlewood Exemption Act (Initial Application)
  • Hazlewood Exemption Act (Renewal Application)
  • Hazlewood Exemption Act (Revocation)

Military Service Graduation Cord Application

• Veterans and active duty members who have applied for graduation with the university are eligible to receive a red, white and blue cord to wear at the graduation ceremony. Please submit the above application. Cords will be available for applicants in the Veterans Affairs Office located in Wimberly 101  the week of graduation or at ceremony check-in.

Military Tuition Assistance/Active Duty Waiver LU Application

• Students receiving Military Tuition Assistance are required to submit the Authorization for Tuition Assistance once approved by their branch of service and the above form each part of term.
